Need dead simple lab case ordering app for dental clinics (replace wufoo)

- We run a commercial dental lab making crowns and bridges for about 45 local dental clinics.
- Right now dentists send case orders via messy paper Rx slips or an old Wufoo form. The form crashes constantly when clinics try to attach 40MB 3D scanner STL files and patient photos.
- Our office manager Brenda is retiring May 12th. She is literally the only person here who can decipher handwritten dentist notes and missing tooth shade numbers. We need this fixed before she leaves.
- Need a clean, no-nonsense web app/portal where clinic staff can log in, select patient tooth numbers on a simple dental arch diagram, pick shade/material from dropdowns, upload scan files, and submit.
- Order needs to generate a clean 1-page printable PDF job ticket with a barcode for our bench technicians to track through casting.
- Tried hiring a guy in November who wasted two months making 50 pretty Figma mockups and never delivered a single working page. Please don't do that. I just need functional software.
- Stack: don't care. Next.js, Supabase, Node, Python, whatever. As long as file uploads to S3 don't timeout on clinic Wi-Fi and it works smoothly on clinic iPads.
- Must be fully tested and deployed before the first week of May.
